Health Leadership Network
The Health Leadership Network was put in motion as the result of a federal grant award. The purpose of the grant is two-fold:
- To build a formal, countywide, collaborative network of health providers with an emphasis on integrating information, administrative, technological, and financial systems across members.
- To utilize the collaborative network to develop the infrastructure necessary to sustain preventive, high quality, easily accessed services for pregnant mothers and families with children birth to age five.
Goals
The five general goal areas addressed in the grant include:
- Prenatal Care
- Oral Health Care
- Mental Health; or healthy attachment and social-emotional development
- Technological linkages; such as a shared Management Information System, Telemedicine, Telehealth, a HLN website, and development of a Health Pass Card
- A vehicle to develop consensus regarding countywide health policies and protocols
Mission and Vision
Mission
The Health Leadership Network will build sustainable systems and infrastructures to improve health outcomes for pregnant women, and children ages birth to five years.
Vision
The Health Leadership Network will become first a regional then a national model for improving health outcomes in rural communities for pregnant women and children ages birth to five years.
Current HLN Membership
Sutter Lakeside Hospital, Adventist Health Redbud Community Hospital, Lake County Department of Health Services, Lake County Office of Education, Lake County Career Center, Lakeside Health Center, Easter Seals, Sutter Lakeside Community Services, and United Way.
